At Patient First Hospice & Palliative Care, we want to ensure our community understands the important relationship between these two types of care.
Palliative care is specialized medical care focused on relieving pain, managing symptoms, and reducing stress for individuals facing serious or life-limiting illnesses. Hospice care is a specific type of palliative care designed for those who are approaching the final stages of life—typically when a physician has determined a life expectancy of six months or less.
While all hospice care is palliative care, not all palliative care is hospice. At Patient First, our primary service is hospice care—a compassionate, holistic approach tailored to meet the physical, emotional, and spiritual needs of patients and their families during this important stage of life.

Our Dementia Care Program is designed to support patients living with Alzheimer’s disease and other forms of dementia through compassionate, person-centered care. We focus on reducing agitation, promoting safety, easing behavioral symptoms, and maintaining dignity at every stage of the disease.

Our Respiratory Support Program offer personalized, comfort-focused interventions to help manage shortness of breath, congestion, anxiety related to breathing, and other respiratory symptoms common at end of life.
This enhanced service ensures patients receive specialized support that helps improve ease of breathing, reduce distress, and promote peace and comfort.

For patients who have limited or no informal caregiving, our Homemaker Program provides essential assistance to help maintain dignity and safety in the home. Homemakers offer light housekeeping, meal preparation, laundry support, linen changes, and home organization to ensure patients remain comfortable in a clean, safe environment. This program is especially valuable for individuals living alone or with limited physical ability.

Understanding that many families struggle with weekend caregiving, we offer enhanced CNA availability on Saturdays and Sundays for patients with minimal or no caregiver backup. Weekend CNAs provide personal care, bathing, repositioning, linen changes, incontinence support, and safety checks to ensure continuous care and prevent caregiver exhaustion or crises.

Rooted in the spirit of Bringing Heartfelt Light, this program honors the stories, memories, and identities of each patient. Through legacy projects, memory totems, storytelling, guided remembrance activities, and our annual Celebration of Life Ceremony, families are supported in preserving their loved one’s legacy with grace and dignity.

Our Peaceful Transition Program provides enhanced, compassionate support during the final days of life. Patients receive daily nurse visits to ensure comfort, manage symptoms, and offer hands-on guidance. Families are supported through increased Social Work and Spiritual Care presence, helping them navigate emotions, find peace, and feel reassured every step of the way. This program ensures that no one faces the final moments alone—surrounded instead by comfort, dignity, and unwavering support.

Our Hospice Medical Director provides clinical oversight and expert guidance for every patient on service.
What our physician does:
The physician ensures care remains aligned with the patient’s goals, values, and comfort.
Our hospice nurses are the heart of patient care. They specialize in expert symptom management, comfort-focused interventions, and compassionate support for both patients and families.
What our RNs do:
Your RN ensures each patient receives consistent, attentive, and personalized care throughout their hospice journey.
Our hospice aides deliver compassionate, dignified personal care and meaningful companionship.
What our CNAs provide:
Your CNA helps patients feel clean, comfortable, and cared for, improving quality of life every day.
Our hospice social workers support patients and families emotionally, socially, and practically during the end-of-life journey.
What our LMSWs do:
Your social worker ensures no family walks this road alone.
Our chaplains provide non-denominational, culturally sensitive spiritual care that honors each patient’s beliefs, traditions, and preferences.
What our Spiritual Care Coordinator offer:
Our Spiritual Care Coordinator meets each person exactly where they are, with respect and compassion.
Our bereavement team supports families before and after the loss of a loved one.
What our grief team provides:
We honor each family’s grief journey with tenderness and understanding.
Our trained hospice volunteers offer companionship, presence, and heart-centered support.
